If you are a low-income medical or cancer patient in the Illinois Valley, you may be eligible for financial support to cover gas, groceries, and comfort care items.

To receive financial aid, you must complete an application and provide:

1) A copy of your treatment plan or doctor’s note.

2) Proof of your low-income status  (Oregon Trail card or SSDI award letter, OHP card, etc.)

Those who qualify will receive:

• Chevron gas cards to assist with the cost of transportation to medical appointments while seeking treatment.

• A gift certificate to be used at a local pharmacy for comfort items.

• Gift cards to help pay for groceries; optional personal shopper delivery service.
